- Select a hypothetical case situation (of an older adolescent or adult individual client) in which you are interested in completing further research. In creating this client situation you need to determine:
The field of social work from which your case will come (e.g. family and
child welfare, health and mental health, corrections, schools, selected disabilities, substance abuse.)
The type of problem the client will have
The type of agency which will provide services to the client (in-patient, out-
patient, etc.)
The types of services provided by the agency
The age of the client - Complete a literature review related to your client situation. Refer to the separate literature review guidelines.
- Create a case on an individual which would be appropriate to your field; do not use an example from the reviewed material. Make up the client’s name as well as a name for your agency. Although you will be focusing on work with an individual, development of your case should illustrate the integration of micro, mezzo, and macro issues into generalist social work practice. The case assessment should be holistic in nature, taking into consideration all aspects of the client’s environment. This includes the client’s family, groups, community, and ethnic/racial memberships. You also need to include the client’s strengths which can be utilized. Your intervention should address the client’s needs at multiple levels.
- Set up a case record in a manila folder, complete with the following:
a. Completed face sheet, altered to meet the needs of your agency but
including the basic information on the agency face sheet.
b. Social history in which you present the data collected on your client. When you write your history, follow the format which is provided.
c. Ecomap placing your client in a social context.
d. A social work assessment.
e. A treatment plan which identifies goals and action plans to reach the goals.
f. The practice evaluation method you will use to assess the effectiveness of the suggested intervention.
