Advanced health assessment

Advanced Health Assessment Critical Evaluation In this assignment you will demonstrate a systematic understanding of an advanced health assessment undertaken on a patient of your choice, with either an undiagnosed or long-term condition. With reference to the underlying pathophysiology of their presenting complaint, you will critically analyse the findings of the consultation in terms of the clinical signs and symptoms exhibited relating to their diagnosis. You should choose a patient who is pertinent to your area of clinical practice, where a clear diagnosis has been determined, following appropriate discussion with your Clinical Supervisor, in order to explore the link between the pathophysiology of the disease process and the physical signs and symptoms you observe throughout the assessment. Assignment format: • 3000 word +/- 10% • Write in third person • Size 12 font, double line space, Ariel or Verdana font • Ensure the report is fully referenced using the Modified Harvard system • Include an appendix – see below • It is recommended that you utilise headings to organise your critical evaluation, as below. Introduction In this section you should indicate the topic and structural content of your assignment. Briefly introduce your clinical role and setting; include reference to the development of Advanced Clinical Practice (ACP) in your role and clinical area, in light of national drivers for clinical service development and ACP competencies. Provide a brief introduction to the patient reviewed with an overview of the underlying pathology to be discussed. It is very important that you maintain both the selected patient’s anonymity and that of the care setting and its staff. Acquisition and Critical Evaluation of Key Clinical Findings Demonstrate a systematic understanding of the history taking process, identifying, analysing and interpreting pivotal points in the consultation which have helped to support or refute the diagnostic decision. Make reference to your structured consultation approach and use of consultation models in your discussion. Critically analyse the physical examination, in light of the known diagnosis, linking any positive clinical signs detected to the proposed pathology. Demonstrate synthesis of knowledge by critically analysing the underlying pathophysiological process occurring which has resulted in the clinical signs exhibited. Make reference to current clinical guidelines and contemporary research to support your discussion. A written example of the clinical noting obtained, ensuring full anonymity, should be included as an appendix to support your discussion, to enable key features of the consultation to be explored in greater depth within the allocated word count. Critical Reflection Critically reflect on the advanced health assessment undertaken, identifying your own learning needs and areas for development. Demonstrate your developing ability to apply problem solving skills to complex problems by reflecting on the approach utilised in the consultation and identify areas where your scope of practice has extended in light of your critical evaluation of the consultation. You may use a Reflection Model of your choice to support your discussion. Conclusion Your assignment should end with a fully referenced conclusion. This draws together what you have said in the main body of the assignment. It should summarise the assessment and analysis you have presented in the main text, without adding any new material.
