Marjorie is a 65 year old female that has recently been diagnosed with colon and lung cancer. Her cancer is highly treatable and patient has expressed desire to live and improve her lifestyle. Her BMI is 45 and she is 100 pounds overweight. She lives a sedentary lifestyle and eats foods of poor nutritional value. She is single, living alone but does have a brother that lives in a neighboring state. She was a previously avid church goer but no longer attends due to mobility and health issues. She currently is on Medicare and has a fixed income.

Approach Plan: Identify which disciplines to include on the team.
Include why you think these disciplines should be included on the team and what contributions they will make to the patient treatment plan.
Make sure to think about the patient as a whole and evaluate the need to include nutritional and spiritual members for example.
Quality of Life and Desired Outcomes: Based upon Marjorie's desire to live and improve her lifestyle. Develop a plan to work with her on setting attainable goals/outcomes.

Education Plan: Based upon the disciplines that are included on the team, develop an education plan that meets Marjorie's needs.
Identify information that will enable the patient to meet each of her specific goals
Examples: Nutrition, exercise, and social aspects of the patient's life
