Discharge Plan

Returning to your client with whom you've completed your video, biopsychosocial assessment, treatment plan, and treatment notes, it's time to discharge him or her from your care.
Using the Discharge Plan format presented in the readings, create a hypothetical discharge plan, assuming that you've completed whatever treatment is indicated, or whatever is appropriate for your Level of Care (you may be referring him or her to a lower or higher LOC, based on progress).
Be sure to talk with your client about her or his expectations of what will happen during termination and post-discharge, and discuss (and document) any referrals.
