Within my work, I exemplify this essential by collaborating with different departments and external organizations. For instance, I noticed a high rate of uncontrolled hypertension among our patients. Instead of just adjusting medications, I worked with our clinic's social work department and a local community center to establish a mobile hypertension screening program. We also created a referral system for patients who needed help with transportation or had food insecurity, as these factors were barriers to consistent care. By addressing the systemic issues outside of the exam room, I was able to improve health outcomes for the entire patient population, not just one person.

 

Evidence-Based Practice (EBP) 🔬

 

Evidence-Based Practice is the DNP essential that integrates the best available research evidence with clinical expertise and patient values to make informed healthcare decisions. It's about moving beyond tradition and intuition to make clinical choices based on credible, up-to-date science.

In my daily practice, I apply EBP by constantly evaluating and updating my clinical protocols. For example, our clinic was using an older guideline for managing Type 2 Diabetes. I researched the latest recommendations from organizations like the American Diabetes Association and found new evidence on the benefits of continuous glucose monitoring and a different class of oral medications. After presenting this research to our clinical team, we revised our protocols. I then used my clinical expertise to tailor these new guidelines to individual patient needs and preferences, such as helping a patient choose a monitoring device that fit their lifestyle. This approach ensures that our patients receive the most effective, cutting-edge care available.
