Teaching Plan Your teaching plan is based on a problem/risk identified during your family health assessment. • Format a nursing diagnosis using Nanda's Nursing diagnosis — see link below. • Assess learner readiness. • Identify a learner goal and 3 measurable goals you want to achieve and the skills you want your family to take away from your teaching presentation.
