Geriatric Nutritional Assessment

1) This nutritional assessment is composed of two parts:
Worksheets: Collect data from your patient (65 yr) using worksheets. The worksheets may be typed or hand-written (must be legible).
Write Up: This is the main body of the paper. It must be typed and conform to APA format.
The library provides guidelines and examples of the most recent APA requirements.
2) This assignment is a required non optional assignment. No late papers will be accepted. The subject of this assessment can be performed on any
patient you have cared for in the clinical setting.
3). Look at your patient’s 24 hour diet history and determine if they are eating a nutritionally balanced diet according to the Food Guide Pyramid. Look
at the percentage of meal intake while hospitalized.
4). Be sure to carefully research your patient’s medical diagnosis and medications. You need to recognize the impact of the medical diagnosis and
medications (if any) on their nutritional status. This should be discussed in your summary and conclusions section.
5). Look at your patient’s anthropometric measurements and compare them to norms.
6). Look at your physical assessment data and compare it to norms.
7). Your paper should have an introductory paragraph and a closing paragraph. The body of
your paper should consist of one paragraph for each section in the paper, (history, physical
assessment, anthropometric data, and lab data)
8). The most important part of your nutritional assessment is the summary and conclusions
section/paragraph. Make sure that you draw conclusions as to whether your patient is meeting their ingestive need. This includes making a nursing
diagnosis, a goal and nursing interventions. Do not simply list data
