Some hints for successful completion of this assignment:
You will need to help develop the data elements to be captured for THREE different data entry screens. Use your imagination to determine what screens might be needed in an ambulatory setting (registration, progress notes, discharge notes etc). In this assignment you are notcreating the screen, but rather compiling the data elements to be included on each of the screens.
For each of the three screens, you must have at least EIGHT data elements that would be captured on the screen. You need to specify the purpose of the data element; specify the format the information will be entered (free text, radio buttons, check box etc); explain why the data element is important to the screen; and why the collection method is appropriate.
You can either summarize your screen elements or you could use a table format to answer the questions.
