Intake Checklist

An important part of the mediation process is gathering information relevant to each case prior to beginning the process. It is likewise important to explain the process to individuals interested in participating in the mediation process.
Imagine you are the intake coordinator for a mediation program (you are the person that receives the calls requesting or inquiring about mediation). You are the first point of contact for these callers. What information should you collect from them? What basic information about mediation should you provide them to explain the process?

Create a 1-2 page “Intake Sheet” that bullets the important pieces of information you should collect from the parties, as well as information you should share with them about the procedures or process of mediation.

Also, if you are directly quoting a source (which should be kept brief and to a minimum) you MUST use quotation marks to indicate that it is a direct quote and not your own reflection on the material. A citation shows the reader where you obtained your data, but if you are using the author's words you must show that with quotes in addition to the citation.
