The Role of the Nurse in Coordinating Care The nurse played a pivotal role in ascertaining that interdisciplinary collaborative care was provided efficiently and effectively. Firstly, the nurse served as a liaison between the patient and the interdisciplinary team, ensuring effective communication and collaboration. They facilitated regular team meetings, allowing members from different disciplines to share their expertise, discuss the patient's progress, and adjust the plan of care as necessary. In this case study, the patient's outcome was met successfully due to the coordinated efforts of the interdisciplinary team. The patient experienced improved respiratory function with the help of respiratory therapy interventions, enhanced mobility through physical therapy exercises, optimized nutrition through dietary modifications suggested by nutritionists, and increased independence facilitated by occupational therapy interventions. Recommendations for Novice Nurse-Leaders As a novice nurse-leader, there are several strategies I would employ to further optimize patient goals in an interdisciplinary collaborative care setting. Firstly, I would emphasize the importance of clear communication among team members, ensuring that everyone is aware of their roles and responsibilities within the care plan. This would involve promoting effective handover practices, encouraging regular team meetings, and implementing electronic health record systems that facilitate seamless information sharing. Secondly, I would advocate for ongoing education and training opportunities for all healthcare professionals involved in interdisciplinary care. By staying up-to-date with advancements in their respective fields, team members can provide evidence-based interventions that maximize patient outcomes. Lastly, I would prioritize patient engagement and empowerment throughout the care process. Involving patients in goal-setting and decision-making empowers them to take an active role in their own care journey. This could be achieved through education sessions, shared decision-making tools, and regular feedback opportunities.
