INSTRUCTION: In this section, you will set the stage for the evaluation project by describing the key health issue
that the health education/promotion program or event addresses. For example, if the program
being evaluated is offered by the American Heart Association, you should discuss the magnitude
of heart-related illness and death. As addressed in HS 3053 and HS 3073, it is helpful to explain
the impact of the key health issue in an inverted triangle fashion (nationally, statewide, and

Program: The Heart Disease Prevention Education Program of Dallas County
Program Mission: To address/correct essential habits that lead to the promotion of heart health.
Goals/objectives of the program:
Program Goal: To reduce the number of individuals aged 45-64 diagnosed with cardiovascular disease in Dallas County. This goal serves to bring those in need (45-64) with community resources that will educate, promote, and help discover preventative measures for decreasing heart disease and promoting heart health.
Process objective: By August 2019, program planners will distribute informational pamphlets on cardiovascular disease prevention and blood pressure check-ups to 10% of Dallas County residents.
Impact objective: After program flyers have been distributed, residents will be able to identify three local blood pressure check-up locations and three modifiable behaviors that prevent heart disease.
Outcome objective: By 2022, heart disease diagnosis will decrease by 10% in Dallas County residents aged 45-64.
