Select a middle range theory and identify application of nursing theories into clinical practice.
Middle-range theories are particularly useful to the Advanced Practice Nurse (APN) because they are more specific than "Grand Theories" (like Orem or Roy) and can be directly tested in clinical settings.
Kolcaba defines comfort as the "immediate state of being strengthened through having the needs for relief, ease, and transcendence addressed in four contexts of experience":
Physical: Pertaining to bodily sensations and immune responses.
Social: Pertaining to interpersonal, family, and societal relationships.
To see how this theory translates into advanced practice, let’s apply it to a patient recovering from a major abdominal surgery.
The APN identifies that the patient is in acute pain (Physical) and is anxious about their incision (Psychospiritual).
Intervention: Administering prescribed analgesics and providing a "splinting" pillow for the patient to hold when coughing.
Theoretical Outcome: The patient achieves a state of "relief," where a specific discomfort is met.
