You work in a rural clinic, and most days you and another PA are the only clinicians in the clinic. A physician is only at the clinic once a week, but is available by phone from another clinic campus out-of-town during your working hours if you need them for consultation.
After talking with the office manager about the proposed policy change, but before you write a written response to your employer about the proposed policy change, you see this patient:
68 y/o F with chronic pain from previous car accident resulting in a 3 level lumbar fusion and peripheral nerve damage. She has been managed by you for the past 3 years on a low dose of Percocet for 2 years and has been compliant with all her drug screening appointments. There has never been a concern of misuse or calling for refills early. She is seeking a refill at this appointment. Her pain is 7/10 when she skips a dose.
