Personal Wellness Plan

How do you practice wellness in your daily life? Are there some domains of wellness that you feel like you are doing well in? Some that could use more work?
Choose 3 domains of wellness to explore these questions.

For each of your chosen wellness domains please describe:

  1. How the domain currently fits into your life
  2. What your priority for change is in the domain
  3. What your goals are for the future and how you plan to implement them

Please use the Wellness Plan Chart to help you organize your thoughts but do NOT submit the chart as part of your response.

• You must include 3-5 research citations
o Please use PubMed to research scientific articles related to your chosen domains.
o You must use APA format for in text citations and reference page
 Please refer to the APA Style Guide folder on Blackboard for help with citations.
• Format
o Response must be a minimum of 4-5 pages long
o Double spaced
o Times New Roman
o 12 pt. font.
o 1 inch margins

• Response must be printed and submitted in class on the due date.
o Emailed responses will not be accepted.
o Late assignments will lose 1 point per day (out of 25 total points).
o Assignments will be collected at the beginning of class.
o Please staple your paper before handing it in.
