Write up a process recording based on one of the scenarios below
1- Pt presents as neutral with a congruent affect. Discussed how she has been adjusting to the unit. Pt reports she is liking the program and hopes it helps her. Pt reports she is nevrous about spending the next few nights at her Mom's because they do not get along. Reports when she is with her Mom they often argue and her Mom puts down her Dad which makes her upset. Writer supported and validated Pt feelings. Writer discussed trying to makes mom house more approachable for her re: having coping skills available for her, having her art supplies and music instruments to use. Pt reports she has all of this. Pt reports she is going to try to see her friends if her Mom allows her. Discussed coming up with an alternative plan if Mom does not allow this. Pt reports she will do art and talk to her friend on the phone. Pt currently denies any thoughts to hurt self or of suicide. Support ongoing.
2- Pt presented as anxious. Pt said she had a "rough weekend" Pt states that she had 4-5 panic attacks this weekend, including one this morning. The panic attack this morning followed after an argument with mom about paying for Pt's AP exam. Pt states that on Friday she got into an argument with her mom. Pt said she asked her mom to bring her guitar over to her dads house and she just put it in her trunk and it broke. Pt said this made her angry because she had that guitar for a long time and it was expensive. Pt said this caused her to "black out" locked herself in the bathroom for an hour. Pt reports that on Saturday her father and older sister had a two hour conversation and PT felt stuck in the middle. Pt says that her father and sister have been arguing lately and it makes everything tense. Pt says that on Sunday her mom spoke to her about getting rid of her dog . Pt said this angered her and she punched a wall. Pt feels that her mom isn't supportive during her panic attacks and she makes them worse by yelling. Pt says her moms family likes to make mental illness seem like its not a problem. Pt said she gets her mom was raised that way, but it doesn't make it right for her to treat the PT the way she does. Pt feels she isn't' a priority to her mom and that she puts her wants over the Pt's needs. PT said she wants to spend an extra day with dad and the court may have to be involved for this. Pt says her mom tries to make her feel guilty for wanting to be around her dad more. Despite having a rough weekend Pt was able to remain safe and did not hurt self. Pt currently denies any thoughts to hurt self or of suicide. Support ongoing.
3- Pt presents as neutral. Discussed weekend progress. Pt states that her weekend was "rough". Pt says her mom, dad and sister had all got into an argument. Pt says that a court date is now set for her to get an extra day at her father's house. Pt says she got into an argument with her mom over this and her phone was taken away because mom said Pt was being disrespectful. Pt says her mom tried to make her feel bad for taking her to court. Pt said she punched a few walls after arguing with mom. On Sunday Pt went to her cousins birthday party and learned to knit from grandma. Pt says she isn't ready to go back to school to deal with teachers and the students. Pt currently denies any thoughts to hurt self or of suicide. Support ongoing.
