Psychiatry. Case history and care planning

      Order Description     Invent a case history and a care plan/support plan for a person with a mental health issue for example psychosis, depression, anxiety disorder, bipolar disorder, eating disorder, self harm. The person lives in his/her own flat in the community. You are his/her support worker supporting him/her to cope with daily life/ social life. Write about the person and the care/support he/she is given. Describe: 1.The persons life history, family, social network, hobbies interests 2. The persons own strengths in coping with his/her illness. 3. The help and support needed for the person to live an independent and meaningful life 4. The symptoms and difficulties that affect the persons daily life/quality of life. 5. Treatment and medication needed to obtain better health. 6. Health professionals and possibly organisations involved in the persons care and how they cooperate holistically 7. How you, the support worker, help and support the person practically, socially and emotionally in his/her daily life
