Safety and Quality Award

Before submitting your project for the safety and quality award, please ensure you have the following documents:

1. Curriculum Vitae (CV) of project lead
2. Project Summary (See questions below)
3. One-page abstract. Promissory abstracts will not be considered. Only abstracts of completed projects will be reviewed. Abstract must include 1) project participant names, 2) project title, 3) objective(s), 4) background, 5) methods, 6) data and results, and 7) conclusion. Individuals with projects in process are encouraged to complete data analysis and apply in subsequent years.

Project Summary

1. What is the patient safety or quality issue?
   [Explain the gap in care that your project focuses on.]
2. What system of change was used to address the problem?
   [What change management methodology was used (e.g., PDSA)? What was the process for implementing the change?]
3. Was the change implemented?
   [Describe the status before and after the change was implemented.]
4. Was the outcome evaluated?
   [What was the desired outcome of implementing the change? Describe the impact the change had on patient care.]
5. How does the safety or quality project affect the greater good of neurology?
   [How does this affect neurology care around the country and world? Can methodology be implemented in other clinics/hospitals to improve care?]
