Background:
Fundin" rel="nofollow">ing opportunities will be available in" rel="nofollow">in the next six months for projects up to two years for the implementation of in" rel="nofollow">innovative preventive and promotional strategies targetin" rel="nofollow">ing diverse population groups at high risk of bein" rel="nofollow">ing effected by diabetes, licit and illicit drug use, cancer, hepatitis, AIDS or cardio-vascular diseases. However, projects, which are not in" rel="nofollow">included in" rel="nofollow">in the above list but are in" rel="nofollow">innovative and can lead to better population health outcomes, may be considered. Partnerships across sectors are strongly encouraged, which may in" rel="nofollow">include hospitals, the crimin" rel="nofollow">inal justice system, children and family welfare groups, community health agencies, Indigenous community-controlled organisations, mental health groups and homeless services. Only projects requirin" rel="nofollow">ing a budget of no more than $300,000 in" rel="nofollow">in total will be considered.
Applicants must demonstrate:
• Attitudin" rel="nofollow">inal and social impact and the cost effectiveness of the project with the target group
• How the project adds value to the health sector; and
• Evidence to demonstrate the need for project and the significance of the approaches strategies proposed in" rel="nofollow">in the project.
Your organisation is plannin" rel="nofollow">ing to submit an application for fundin" rel="nofollow">ing to address one or a few of the problems identified in" rel="nofollow">in the last annual review. Before the project plan and application can be developed, the project development group of your organisation is assigned to conduct a needs analysis to in" rel="nofollow">inform the formulation of project ideas and the development of the project proposal.

have been efforts to address the problems. If yes, what is the current service capacity in" rel="nofollow">in addressin" rel="nofollow">ing
the problems and what are the gaps for further in" rel="nofollow">interventions or services? If no, you need to discuss
why these problems have not been addressed by current efforts/services.
