  1. Address the steps of the change theory and apply how change will be created for your program to be successful.
    Identify your Program Planning Team’s roles and responsibilities for each of the change steps.
  2. Describe the outcome measures that you will use to address whether change has been successful and your program has been integrated in your community region of responsibility.
  3. Design a schematic diagram demonstrating the steps of the change effort that your planning team has addressed.
